Billing Information

 

Appointments are billed to the patients' insurance and the patient is responsible to cover any other costs not covered by their insurance on the day of the appointment. We provide an estimated cost based on your plan's deductible copay and out of pocket maximum. All estimates are subject to change. If you are unable to pay the full amount at the time of the appointment, payment plan options are available. Please see one of our practice managers to set up a payment plan or contact (304) 554-0504.
